For your convenience, print this page and follow the steps in the order presented

Step1: Collecting US passport application materials:

1. Proof of US citizenship: (one of the following)

1. Original or Certified copy of Birth Certificate (if born in the US)

2. Old passport of which the applicant was the bearer.

3. Original Naturalization Certificate (US citizens through naturalization)

4. Certificate of Citizenship (if born abroad to US parent/s)

2. Parents Proof of Identity (very important)

Valid driver’s license with photo issued more than 6 months ago.

»

Make a clear photocopy of driver’s license with photo on it.

» If parent does not possess a valid photo driver’s license or if license was issued LESS than 6 months ago (even renewals), click for additional proof of Identity Instructions.

3. Child’s Relationship to parent(s)/Guardian (one of the following)

1. Original or Certified Copy of Birth Certificate.

2. Foreign Birth Certificate (with parents names & official translation)

3. Adoption Decree (with adopting parents names)

4. Court order establishing custody.

5. Court order establishing Guardianship

4. Parental Application Permission Documentation

  1. Both parents must appear together OR

  2. One parents appear and submits second parent’s written statement of consent authorizing passport issuance for the child OR

  3. One parents appear and submits primary evidence sole authority to apply:

    » Original or Certified copy of Birth Certificate listing only applying parent.

    » Foreign Birth Certificate (official translation) listing only applying parent.

    » Court Order granting sole custody to applying parent

    » Adoption decree, if applying parent is sole adopting parent

    » Death certificate of non-applying parent.
» If none of the above documentation is available, the applying parent/guardian should submit a signed statement explaining why the non-applying parent/guardian consent cannot be obtained.

5. Two Passport Photos

6. Letter of Authorization

» Click for Child Letter of Authorization, print 2 copies

» This letter allows Visa Obtainers, Inc. to expedite your passport application.

7. DS-11: Application for Passport (click link)

  • Click on the link above to access the electronic DS-11.
  • You MUST fill out this form electronically and then print 2 copies
» Fill out all of the fields on the form.

» Click on the "create form" button to generate the completed form.

» Print at least TWO copies of the form.

» Do not sign the form until you appear in front of your local passport Acceptance Agent

8. Proof of Travel (one of the following)

Copy of travel itinerary from the travel agent or airline.

»

Copy of plane ticket.

» If travel is for business and neither of the above is available, provide a business travel letter, click for sample business travel letter

9. Fees (government fees and service fees are separate)

» Government fee: 2 checks or money orders:

  1. One check or money order to US Passport Services for $ 112, please include applicant name and date of birth on the check or money order.

  2. Second check or money order for $ 30 to your local Passport Acceptance Agent (AA) as explained in step 2 below. Some (AA) collect $ 30 cash.

Service fee: A check or money order to Visa Obtainers, Inc. for $ 95

» Shipping fee: Add $ 18 to the service fee OR include a pre-paid FEDEX airbill for the overnight return of your passport.

10. Passport Order Form

Step 2: Authorization Process
»

Present the following items IN PERSON to your local Passport Acceptance Agent (AA):

  1. Proof of US citizenship.
  2. Proof of child relationship to parents
  3. Parental permission document
  4. Passport application form DSP-11 completed but not signed as the Acceptance Agent must witness the parent/s signing the application form, applicants 13 and under do not sign DSP-11 or need personal appear before (AA) with parent(s)
  5. Two passport photographs.
  6. Parent proof of identity.
  7. 1st copy of authorization letter.
  8. Government fees: $ 142 ($30 cash to AA + $ 112 check or money order to US Passport Services)
»

(AA) are in Passport Acceptance Facility in local Post Offices or Court Houses.

» Notify (AA) that a courier service will deliver the passport application materials to a Passport Agency.

» (AA) endorses DSP-11 and seals the documents in a Government envelope. Do NOT open envelope after it is sealed.

» These materials are NOT sealed in the government envelope:

1. 2nd copy of letter of authorizations
2. Proof of travel
3. Service fee
4. Passport order form

»

On outside the Sealed envelope print applicant’s name, departure date and date of birth.



Step 3: Ship application materials via Federal Express
»

Call 1-800-977-2909 to place an order and receive shipping instructions with the following ready:

1. Government envelope.
2. 2nd copy of letter of authorization
3. Passport order form.
4. Proof of travel.
5. Visa obtainers, Inc. service fee.
