Step1: Collecting US passport application materials:
1. Proof of US citizenship: (one of the following)
1. Original or Certified copy of Birth Certificate(if born in the US)
2. Old passport of which the applicant was the bearer.
3. Original Naturalization Certificate (US citizens through naturalization)
4. Certificate of Citizenship (if born abroad to US parent/s)
2. Parents Proof of Identity (very important)

Valid driver’s license with photo issued more than 6 months ago.

Make a clear photocopy of driver’s license with photo on it.

If parent does not possess a valid photo driver’s license or if license was issued LESS than 6 months ago (even renewals), click for additional proof of Identity Instructions.
3. Child’s Relationship to parent(s)/Guardian (one of the following)
2. Foreign Birth Certificate (with parents names & official translation)
3. Adoption Decree (with adopting parents names)
4. Court order establishing custody.
5. Court order establishing Guardianship
4. Parental Application Permission Documentation
Both parents must appear together OR
One parents appear and submits second parent’s written statement of consent authorizing passport issuance for the child OR
One parents appear and submits primary evidence sole authority to apply:
» Original or Certified copy of Birth Certificate listing only applying parent.
» Foreign Birth Certificate (official translation) listing only applying parent.
» Court Order granting sole custody to applying parent
» Adoption decree, if applying parent is sole adopting parent
» Death certificate of non-applying parent.

If none of the above documentation is available, the applying parent/guardian should submit a signed statement explaining why the non-applying parent/guardian consent cannot be obtained.
Present the following items IN PERSON to your local Passport Acceptance Agent (AA):
Proof of US citizenship.
Proof of child relationship to parents
Parental permission document
Passport application form DSP-11 completed but not signed as the Acceptance Agent must witness the parent/s signing the application form, applicants 13 and under do not sign DSP-11 or need personal appear before (AA) with parent(s)
Two passport photographs.
Parent proof of identity.
1st copy of authorization letter.
Government fees: $ 142 ($30 cash to AA + $ 112 check or money order to US Passport Services)
