Step1: Collecting US passport application materials:

1. Your one year extendable passport.

• This instructions can only be use if your passport is still valid.
• If the Passport has expired you would need to follow the new passport instruction

2. Proof of US citizenship: (one of the following)

1. Original or Certified copy of Birth Certificate (if born in the US)

2. Old passport of which the applicant was the bearer.

3. Original Naturalization Certificate (US citizens through naturalization)

4. Certificate of Citizenship (if born abroad to US parent/s)

3. Clear photocopy of driver’s license with photo on it.

4. Letter of Authorization

»	This letter allows Visa Obtainers, Inc. to expedite your passport application.

5. US Passport Application Form DS-5504 completed and signed

»	One copy of the government form DS-5504 (Application additional VISA page). Click on the link above to access electronic DS-5504. Print the application DS-5504. Do not forget to sign after you have printed completed online form. Call us if you have any question about this form 1-800-977-2909
»	Faxed forms are NOT acceptable.

6. Proof of Travel (one of the following)

»	Copy of travel itinerary from the travel agent or airline.
»	Copy of plane ticket.
»	If travel is for business and neither of the above is available, provide a business travel letter, click for sample business travel letter.

7. Fees

»	Government fee: A check or money order to US Passport service for $ 60 Write your name and date of birth on the check or money order.
»	Service fee: A check or money order to Visa Obtainers, Inc. for $ 75
»	Shipping fee: Add $ 18 to the service fee OR include a pre-paid FEDEX airbill for the overnight return of your passport.
»	If you are paying by credit card, please use the credit card payment information on the Passport order form
