Step1: Collecting US passport application materials:

1. Expired or Expiring Passport

» This passport will be return with your new one.

2. Two Passport Photos

» Click for Passport Photos Regulations.

3. Letter of Authorization

»	Click for Letter of Authorization, print 1 copy
»	This letter allows Visa Obtainers, Inc. to expedite your passport application.

4. One Copy of the Government DS-82 - "Application for Passport By Mail"

• Click on the link above to access the electronic DS-82.

»	Confirm that you meet the requirements listed, and click "continue".
»	Complete the form electronically and click "create form".
»	Print the resulting document.
»	Close the document.
»	Do not forget to sign the document once it has printed.

5. Proof of Travel (one of the following)

»	Copy of travel itinerary from the travel agent or airline.
»	Copy of plane ticket.
»	If travel is for business and neither of the above is available, provide a business travel letter, click for sample business travel letter.

6. Fees

»	Government fee: A check or money order to US Passport service for $ 127 Write your name and date of birth on the check or money order.
»	Service fee: A check or money order to Visa Obtainers, Inc. for $ 75
»	Shipping fee: Add $ 18 to the service fee OR include a pre-paid FEDEX airbill for the overnight return of your passport.
»	If you are paying by credit card, please use the credit card payment information on the Passport order form

7. Passport Order Form

»	Call 1-800-977-2909 to place an order and receive shipping instructions with the following ready:

1. Expired or expiring passport.
2. Application form DS-82 completed and signed.
3. Letter of authorization
4. Clear photocopy of driver’s license.
5. Government fee: $ 127 a check or money order
6. Passport order form.
7. Proof of travel.
8. Visa obtainers, Inc. service fee.
