Step1: Collecting US passport application materials:

1. Your valid US passport

» This passport will be return with your new one.

2. Letter to US Passport Services

» This letter must state reason for needing a second valid US passport.

3. Two Passport Photos

4. Letter of Authorization

»	Click for Letter of Authorization, print 1 copy
»	This letter allows Visa Obtainers, Inc. to expedite your passport application.

5. One Copy of the Government DS-82 - "Application for Passport By Mail"

• Click on the link above to access the electronic DS-82.

»	Confirm that you meet the requirements listed, and click "continue".
»	Complete the form and click "create form".
»	Print the resulting document
»	Do not forget to sign the document once it has printed.

6. Proof of Travel (one of the following)

»	Copy of travel itinerary from the travel agent or airline.
»	Copy of plane ticket.
»	If travel is for business and neither of the above is available, provide a business travel letter, click for sample business travel letter.

7. Fees

»	Government fee: A check or money order to US Passport service for $ 127 Write your name and date of birth on the check or money order.
»	Service fee: A check or money order to Visa Obtainers, Inc. for $ 95
»	Shipping fee: Add $ 18 to the service fee OR include a pre-paid FEDEX airbill for the overnight return of your passport.
»	If you are paying by credit card, please use the credit card payment information on the Passport order form

1. Your valid US passport.
2. Two Passport Photographs
3. Application form DSP-82
4. Letter of authorization
5. Letter to US passport Services.
6. Clear photocopy of driver’s license.
7. Government fee: $ 127 a check or money order
9. Proof of travel.
10. Visa obtainers, Inc. service fee.
